COVID-19 Disclaimer
As lawful consideration for participation in golf activities (including, e.g., playing golf, driving golf carts, attending the banquet dinner, and using golf facilities and equipment (the “Activities”) at Bliss Creek, I hereby represent, acknowledge and agree as follows: I am not currently restricted from engaging in the Activities or attending the Club under the State of Illinoi Shelter in Place Order due to my age, condition or for any other reason. I am aware my participation in the Activities and my presence at Bliss Creek and/or golf course will cause me to be near and/or in contact with Club staff, patrons and other persons, which raises the risk to me and others of exposure to COVID-19, and I hereby agree, on behalf of myself and my heirs, to forever waive, release, discharge, indemnify and hold harmless the Warrenville Lions Club, any member of the Warrenville Lions Club, Lions Club International, and Bliss Creek, as well as each of their respective owners, affiliates, subsidiaries, directors, officers, agents, employees, representatives, successors and assigns, and any other parties acting on their behalf (collectively, the “Released Parties”) from any and all claims, liabilities, damages, expenses, and costs, including, but not limited to, attorneys’ fees and costs arising out of or in connection with exposure to COVID-19 that may result in illness, injury, and/or death, which may now or hereafter be brought as a result of my participation in the Activity. I acknowledge that I am voluntarily participating in the Activities and visiting the golf course and Bliss Creek facilities with an express understanding regarding the coronavirus pandemic and the dangers described above, and I hereby agree to accept and assume any and all risks associated therewith.
By purchasing a ticket, attending, and/or otherwise participating, I acknowledge that I have read and fully understood all of the terms of this Disclaimer, I agree to follow all COVID-19 Protective Measures of the Club, and I am voluntarily giving up potential legal rights, including the right to sue the Released Parties.
